Many discrepancies in info on John Reid McIntire Sr's Certificate Of Death, issued June 10, 1998, by the Department of Public Health Fort Wayne, Allen County, Indiana 46802. Health Commissioner - Registrar's signature is not clear but appears to be something like - Jane M. Irmscher M.D. The Registar - Vital Statistics Division appears to be a Janet A. Anderson.
Death Certificate has age as 70, birthdate as September 26, 1865 and date of death as December 14, 1920 - 55 years after stated birthdate. Pretty obvious discrepancy. Obituary published in Winchester, Indiana (where his 2 daughters resided) Dec 1920 has that he died in a Ft Wayne hospital and confirms his age as 70.
Death certificate lists place of burial as Winchester, Indiana. The obituary states there will be visitation and a service at the home of one of his daughters in Winchester but burial will take place at his former home of Dayton, Ohio. Cemetery records at Woodland Cemetery in Dayton show he was buried there Dec 1920 next to his wife Mary, who had died in 1906, and that his age at time of death was 70. But their records indicate he died in Winchester, Indiana.
Death certificate lists his birthplace as WINCHESTER, his father as JOHN and his father's birthplace as INDIANA. To date I have not been able to locate any piece of information that will support any of that. For Mother and Mother's birthplace, NOT STATED was entered at both locations.
I was disappointed with the death certificate provided by Allen County, Indiana, particularly after being subjected to the process they put you through in order to obtain one.
